Horizon Medical Clinic is an Urgent Care and Walk-In Clinic facility that provides convenient, immediate medical care for non-life-threatening illnesses or injuries that require prompt attention but do not necessitate a trip to the emergency room.
We provide primary care to individuals and families which include preventive and wellness measures to help improve overall health. We are dedicated to providing high-quality and affordable health care for every patient in the community with our diverse set of care and medical services.

How Do I schedule an appointment?
To schedule an appointment, you can call our clinic directly at (321)-363-3384 or book an appointment on our website. We recommend making an appointment in advance, but we also accept walk-ins based on availability. If you need to be seen on the same day, please explain your situation when you call, and we will do our best to accommodate you.
You can now schedule an appointment for either an in-person or zoom telemedicine meeting. Once scheduled, and confirmed, we will reach out to you with zoom meeting details and information.
If you need to cancel or reschedule your appointment, please do so at least 2-Hours prior to the scheduled time. Please view our Terms of ervice for further details.

How Do I refill my prescription?
To refill your prescription, please contact our office directly. You may also reach out to your pharmacy; they can often handle the refill request on your behalf.
Do you offer Telehealth?
Yes, we offer telemedicine services for certain types of appointments. Please inquire when scheduling your appointment to see if telemedicine is appropriate for your needs. You must have a computer, laptop, tablet or phone with a speaker/mic and built-in camera, web camera or attached camera, in order for the Zoom meeting to function.

We have partnered with Tebra to provide you a convenient patient portal once you have registered with us. Through the portal you can do the following:
- View easy-to-read patient statements
- Make fast, secure online payments
- Review your health information
You can access your medical records by submitting a “Medical Release of Information” form at our clinic’s front desk. Once the form is completed, we will prepare your records for you.
